Evergreen Chapter CMAA 29th Annual Golf Tournament Entry Form Monday, September 30th at Tumble Creek Golf Course at Suncadia
$1000 per foursome, $250 per player.
Registration includes lunch prior to golf, hors d'ouevres after
play concludes, three raffle tickets per player (a total value of $200 per team), powered carts, and on-course competitions.
